CFSS Community First Services & Support

As of October 1, 2024, PCA was replaced by Community First Services and Supports CFSS.

 Community First Services and Supports (CFSS) is a Medicaid program designed to provide flexible and individualized services and supports to people with disabilities and older adults. The primary goal is to empower individuals to live independently in their own homes and communities, rather than in institutional settings. CFSS aims to enhance the quality of life, promote self-determination, and ensure that people have the necessary resources to thrive. 

In this model, a CFSS agency (like Your Journey Home Care Solutions) handles most of the responsibilities for you.

• Hire, train, and manage caregivers
• Handle scheduling and supervision
• Take care of payroll, taxes, and background checks
• Provide backup staff if your usual caregiver is unavailable
• Monitor services to ensure quality and compliance
   

• Help shape your care plan
• Communicate needs and preferences
• Choose from the agency’s caregiver pool (or recommend someone)

• Individuals who want support and less responsibility
• Families new to caregiving or needing professional oversight
   

This model gives you more control and flexibility. You manage your own care services using a personal budget approved by the state.

• Hire and train your own caregiver(s)—even a family member (with some restrictions)
• Set schedules and supervise care
• Work with a financial management service (FMS) that handles payroll and taxes
• Manage your budget for staffing, support, and other approved goods/services
   

• Take full responsibility for managing your care team
• Maintain documentation, timesheets, and service reports
• Direct your support within budget limits
   

• Individuals who want maximum independence
• People comfortable with hiring and supervising staff
• Those who already have a trusted caregiver in mind
