Frequently Asked Questions

Community First Services and Supports (CFSS) is a Minnesota program that provides personal care and support services for individuals who need help with daily activities due to a disability, medical condition, or age-related need. It promotes independence and gives you choice and control over your care.

PCA (Personal Care Assistance) is the older model. CFSS is the updated program that offers more flexibility and allows you to hire your own caregiver, including family members (with a few exceptions), and gives you more say in your care plan.

You may qualify if you:

• Receive Medical Assistance (Minnesota Medicaid)
• Need help with daily activities such as dressing, bathing, or mobility
• Complete a MnCHOICES assessment through your county

Yes! CFSS gives you the ability to select and train your own caregiver—whether it's a friend, family member, or someone from your community. Or you can choose one of our trained, trusted caregivers.

MnCHOICES is a comprehensive, in-person or virtual assessment conducted by your county. It evaluates your ability to perform daily living tasks and identifies the supports you may need to live safely and independently at home.

Contact your county human services office and request a MnCHOICES assessment. If you need help, Your Journey Home Care Solutions can assist you with contacting the right department.

If you are found eligible, you will:

• Receive a service authorization 
• Choose a CFSS provider agency (like us!)
• Work with a consultation services provider to develop your care plan
• Begin services once your plan is approved
