Community First Services and Supports (CFSS) is a Minnesota program that provides personal care and support services for individuals who need help with daily activities due to a disability, medical condition, or age-related need. It promotes independence and gives you choice and control over your care.

 PCA (Personal Care Assistance) and CFSS (Community First Services and Supports) are Minnesota programs that provide support with daily activities. CFSS offers additional options, including an agency model and a budget model, giving eligible individuals more flexibility in directing their services. 

 Eligibility for CFSS or PCA services depends on Minnesota program requirements and an individual assessment. Eligible programs may include Medical Assistance (MA), Alternative Care (AC), certain MinnesotaCare programs, and some HCBS waiver programs. An assessment helps determine whether a person needs assistance with daily activities and what services may be available. 

 Yes. CFSS gives you a voice in selecting your support workers. Under the Agency Model, the CFSS provider agency is the employer, but you can provide input on worker selection and can decline workers or request new workers. Under the Budget Model, you are the employer and are responsible for recruiting, hiring, training, and supervising your workers. Family members, including a spouse or parent of a minor, may be able to provide CFSS services when program requirements are met. 

 A MnCHOICES assessment is an assessment used to determine eligibility for CFSS and identify the services and supports a person may need. The assessment looks at your health, daily activities, support needs, and ability to direct your care. A lead agency, such as your county or Tribal Nation, completes the assessment.

 After your assessment, if you are eligible for CFSS, you will work through the next steps based on your chosen service model. This may include choosing a consultation services provider, selecting the Agency Model or Budget Model, developing your CFSS service delivery plan, and receiving approval and authorization for your services.
