CFSS Agency Model

 The CFSS Agency Model allows you to receive services through a participating CFSS provider agency. The agency employs and manages your support workers while you direct your care and help choose your workers. 

CFSS Budget Model

 The CFSS Budget Model gives you more responsibility and control over your approved CFSS budget. You employ your support workers and work with a financial management services (FMS) provider for employer-related support. 

Who May Qualify for CFSS?

 CFSS may be available to Minnesotans who meet Minnesota program requirements and have an assessment that identifies a need for supports. Eligibility may depend on the individual’s program, financial eligibility, assessment results, and service authorization.
